Blood taken directly from the adrenal vein gave a positive result, but under deep morphinization the blood from the adrenal vein was negative, and under deep morphinization the foregoing adequate stimuli were negative. The one agent which was found to protect the brain against changes in the Nissl substance morphin gave a negative result in the Cannon test for adrenalin.
In our extensive study of the brain in its relation to the production of energy and the consequent exhaustion caused by fear and rage; by the injection of foreign proteins, of bacterial toxins, and of strychnin; by anaphylaxis; by the injection of thyroid extract, of adrenalin, and of morphin, we found that, with the exception of morphin, each of these agents produced identical changes in the brain-cells.
The main indication in treatment is to secure sleep, and this is done by the administration of bromides, chloral, or paraldehyde, or of one or other of the drugs of which sulphonal, trional, and veronal are examples. If for any reason morphin is contraindicated, and if pain is not a symptom, the patient's nerves may be quieted and rest may be given by sodium bromid, or by veronal-sodium, the dose of the former being 2 gm.
Hæmorrhage is arrested in the limbs by an improvised tourniquet; in the head by a pad and bandage; in the thorax or abdomen by packing if necessary, but this should be avoided if possible, as it favours septic infection. If a limb is all but detached it should be completely severed. A full dose of morphin is given hypodermically.
On the other hand, in animals in which an acidity had already been produced by ether, by shock, by anger, or by fear, the later administration of morphin delayed or inhibited entirely the neutralization of the acidity.
If pain is not severe, or due to inflammatory distention of some undilatable part, to pressure on some nerve, to distention of some tube by a calculus or to some serious injury to the nerves, large doses of morphin are not needed. Small doses will act much more efficiently.
Almost coincidently with the administration of nitroglycerin or the amyl nitrite, a hypodermic injection of 1/8 or 1/6 grain of morphin sulphate should be given without atropin, as full relaxation is desired without any stimulation of atropin. Alcohol is also a valuable treatment of this pain, when the drugs mentioned are not at hand.
In the case observed by him the patient took laudanum for six years in increasing doses up to one ounce per day; for eighteen months, pure opium, commencing with 15 grains and increasing to 2 1/4 drams daily; and for eighteen months morphin, in commencing quantities of six grains, which were later increased to 40 grains a day.
Jones of New Orleans relates the case of a girl of twenty-seven who had been asleep for the last eighteen years, only waking at certain intervals, and then remaining awake from seven to ten minutes. The sleep commenced at the age of nine, after repeated large doses of quinin and morphin.
