Before analyzing such influence it is important to notice two other causes of menorrhagia, that are very frequently present in just such cases as Dr. Clarke describes. These are prolonged sedentary position, and deficiency of physical exercise. Either may determine anemia, or impoverishment of the blood, a condition which alone is sufficient to induce excessive menstrual flow.
The child puts his hand in the fire; physical injury pain results, and the appropriate muscular response is elicited. If pressure is prolonged on some parts of the body, anemia of the parts may result, with a corresponding discomfort or pain, requiring muscular action for relief.
All high fever increases the heart's action, but not so markedly in typhoid fever as in other fevers; in fact, the heart in typhoid fever, during the early stages, is apt to be slower than the temperature would seem to call for. In anemia when the patient is active the heart is generally rapid. The rapid heart from cardiac disease has already been considered.
This activity results in an increase of the body heat and in an anemia of the superficial parts of the body, so that less heat can be lost by radiation. By this means, therefore, the external portions of the body contribute measurably to the production of the beneficent and saving fever.
Even more puzzling, though, was the final case which put an end to the renting of the house a series of anemia deaths preceded by progressive madnesses wherein the patient would craftily attempt the lives of his relatives by incisions in the neck or wrist.
Such a patient may arise from his bed thinking that he is cured; but unless he is afterward treated by natural methods, he will never make a full recovery. It will take him, perhaps, months or years to die a gradual, miserable death through malassimilation and malnutrition, which usually end in some form of wasting disease, such as pernicious anemia or tuberculosis.
The hair becomes shaggy and rough, losing all luster, and tends to grow irregularly and fall out. The temperature becomes subnormal and an anemia supervenes. In the form of hemoglobin this element is the chief agent in carrying oxygen from the lungs to the tissues of the body. In the manufacture of foods, much of the iron is lost. For instance, whole wheat flour contains about ten times as much iron as does the white flour. Too little iron causes, among other ills, anemia, and if the iron is very low, chlorosis or the green sickness may ensue.
It may be caused by the condition of the blood, whether it be impoverished anemia or too rich in red globules; by reflex irritation of the pneumogastric or sympathetic nerves; by overexertion; or by alcoholism. It may also be due to general debility; the woman resists fatigue less easily, and she experiences a general malaise.
And yet it was amazing the amount of observation, and insight, and solid concentrated thought the young man contrived to pack into those five minutes. Well it seemed that it was not general paralysis this time, nor yet anemia of the brain; but he could tell her more about it in the morning.
