The diagnosis is easy if the patient or his friends are aware of the family tendency to hæmorrhage and inform the doctor of it, but they are often sensitive and reticent regarding the fact, and it may only be elicited after close investigation. From the history it is usually easy to exclude scurvy and purpura.

The Purpura employed seems to have been the P. lapillus, a mollusc not confined to the Mediterranean, but one which frequents also our own shores, and was once turned to some account in Ireland. The varieties of the P. lapillus differ considerably. Some are nearly white, some greyish, others buff striped with brown. Some, again, are smooth, others nearly as rough as the Murex trunculus.
The Hermit-crab cannot do this, and when his dwelling has become too narrow he abandons it for one that is more comfortable. At first enclosed in the remains of a Trochus, he changes into that of a Purpura; a little later he seeks asylum in a Whelk.
The existence of accidental nigrities rests on well-established facts which are distinctly different from the pigmentation of purpura, icterus, or that produced by metallic salts. Chomel quotes the case of a very apathic old soldier, whose skin, without any appreciable cause, became as brown as that of a negro in some parts, and a yellowish-brown in others.

Cuming and Hinds have found, on the comparison of nearly two thousand living species from the opposite sides of South America, only one in common, namely, the Purpura lapillus from both sides of the Isthmus of Panama: even the shells collected by myself amongst the Chonos Islands and on the coast of Patagonia, are dissimilar, and we must descend to the apex of the continent, to Tierra del Fuego, to find these two great conchological provinces united into one.
