The defective teeth, septic tonsils, discharging adenoids, poverty of their parents and their own laziness, all conspire to cause digestive troubles which bear a fruitful crop of further evils, for thus are caused such illnesses as anæmia and gastric ulcer. Constipation claims a few words to itself. And here again we ought to consider certain septic processes.
As in all septic processes, the nutrition must be pushed to the full extent to which it can be tolerated by the patient, namely, small amounts of a nutritious, varied diet given at three-hour intervals.
In this case the microbe of pus carried so to speak by the septic vibrio, accompanies it throughout the body: the highly-inflamed muscular tissues, full of serous fluid, showing also globules of pus here and there, are like a kneading of the two organisms.
It may have been due to incipient scurvy, or to the nature of the rations, or to the general state of health, or it may have been caused by some septic condition of the mouth, induced by the heat and dryness. Some young fellows lost every tooth in their possession in a year. Hair suffered in the same way, but to a lesser extent.
Wounds invading the tissues adjacent to joints, when these wounds are of considerable extent, cause inflammation of such articulations by contiguous extension of inflammation. As long as an injury remains practically aseptic, or if infected and the septic process does not involve the joint proper by direct extension, no more serious disturbance than a simple synovitis will result.
That terrible attack of septic pneumonia in New York, as we now know, left the heart injured and the lungs weakened. He was badly nursed, and his state of mind at the time his misery and loneliness left him little chance. Then the drinking habit, which he contracted during those wretched months in the States, has been of course sorely against him.
I soon found that the man's arm was not only septic, but broken and splintered. "Got a pair of scissors?" I asked. One man had a pair of nail-scissors, and with this very awkward instrument I proceeded to operate. It was a terrible gash. His sleeve was soaked in blood. I cut it away, and his shirt also. I broke an iodine phial and poured the yellow chemical into his great gaping wound.
The rapid onset of the conditions is rather unusual, but may be explained if we regard the case as a mild and unnoticed diphtheria, subsequently complicated by paralysis and by secondary septic infection, for which reasons she came under observation. 'In Thasos, the wife of Delearces who lodged on the plain, through sorrow was seized with an acute and shivering fever.
If the induration is well marked, the chancre can be palpated through the prepuce, and is tender on pressure. As under these conditions it is impossible for the patient to keep the parts clean, septic infection becomes a prominent feature, the prepuce is œdematous and inflamed, and there is an abundant discharge of pus from its orifice.
To be able to distinguish between the generalization of a septic infection in its incipiency, and a more or less benign edema, is largely possible by digital manipulation alone. An extremity may be greatly swollen because of the existence of chronic lymphangitis, influenza, or an acute septic infection occasioned by the introduction of pathogenic and aerogenic organisms.
