It may be a foreign body, such as a piece of dead bone, an infected ligature, or a bullet, acting mechanically or by keeping up discharge, and if the body is removed the sinus usually heals. The presence of a foreign body is often suggested by a mass of redundant granulations at the mouth of the sinus.
When these and other non-operative measures fail, and the whole track of the sinus is accessible, it should be laid open, scraped, and packed with bismuth or iodoform gauze until it heals from the bottom. The tuberculous ulcer is described in the chapter on ulcers. Definition. Virus.
A sinus is lined by granulation tissue, and when it is of long standing the opening may be dragged below the level of the surrounding skin by contraction of the scar tissue around it. As a sinus will persist until the obstacle to closure of the original abscess is removed, it is necessary that this should be sought for.
The angle of the anterior chamber is a true angle and not an annular sinus. The individual strands of this meshwork are more than two times as large at advanced age as at birth, consequently the alveoli of the meshwork becomes markedly reduced in size.
The flexor tendons are sometimes destroyed, the toe being dorsiflexed by the unopposed extensors. The depth of the track being so disproportionate to its superficial area, the condition closely simulates a tuberculous sinus, for which it is liable to be mistaken.
A very little way inland is the village of Dunsley, which may have been in existence in Roman times, for Ptolemy mentions Dunus Sinus as a bay frequently used by the Romans as a landing-place. The foundations of some ancient building can easily be traced in the rough grass at the village cross-roads, now overlooked by a new stone house.
Fischer in a personal letter says: "You have two possibilities for the production of glaucoma with sinus disease: A toxic factor due to poisons being carried into the eye; and second, interference with a proper blood supply to the eye through compression of the efferent or afferent blood vessels supplying the eye from edema of the tissues about the eye consequent upon the sinus infection.
Miletus lay on the southern shore of a deep bay the Sinus Latmicus which penetrated the western coast of Asia Minor in about Lat. 37º 30', but which the deposits of the Maeander have now filled up. North-west of the town, at the distance of about a mile, was the small island of Lade, now a mere hillock on the flat alluvial plain.
Hence the suggested association of the dental surgeon with the ophthalmologist in these cases of apparently idiopathic increased intra-ocular tension. It would be well to state here a cursory examination of the mouth will not discover root abscesses any more than such examination will discover non-suppurative sinus disease.
Most animals are not affected by the cold weather if given dry quarters and a dry bed. Irritation to the mucous membrane by dust, gases and germs is a common cause. Influenza and colt distemper are characterized by an inflammation of the respiratory mucous membranes. In the horse, chronic catarrh is commonly caused by diseased teeth, and injuries to the wall of the maxillary sinus.
