When there is hypotension, or when the diastolic pressure is high and the venous pressure is high, and when there is edema or effusion, watery catharsis should be caused only after due consideration, and always with a careful watching of the effect on the heart and blood pressure. The blood pressure is lowered by such catharsis, and the heart is often slowed.

An active purgative should be given at once and in the ordinary case, stimulants are indicated. If marked distress is present, morphin is given and where there is much rise of temperature, cold drinking water is offered in abundance and catharsis is enhanced by enemata. Locally, hot applications are of benefit.

If the bowels are known to be in excellent condition and not loaded with fecal matters, brisk catharsis is not needed simply because endocarditis has developed. If the bowels have been neglected, a small dose of calomel, aided by a compound aloin tablet, is necessary and good treatment.

While generally toxins from intestinal indigestion make a heart irritable and more rapid, sometimes they slow a heart, and in such cases the heart will be improved when catharsis has been caused and a modification of the diet is ordered.

Catharsis is rather the consciousness of how evil evils are, and how besetting; and how possible goods lie between and involve serious renunciations. To understand, to accept, and to use the situation in which a mortal may find himself is the function of art and reason.

A tragedy, then, is the imitation of an action that is serious and also, as having magnitude, complete in itself; in language with pleasurable accessories, each kind brought in separately in the parts of the work; in a dramatic, not in a narrative form; with incidents arousing pity and fear, wherewith to accomplish its catharsis of such emotions.

As endocarditis is generally secondary to some acute disease, the patient has become weakened already, and the circulation is not sturdy; therefore such a drug as aconite is probably never indicated. The necessary diminished diet, catharsis, hypnotic, salicylic acid and alkalies all tend to quiet the circulation and diminish any strenuosity of the heart that may be present.

Thus a man whose physiological complexion involves more poignant emotion than his ideas can absorb one who is sentimental will yearn for new objects that may explain, embody, and focus his dumb feelings; and these objects, if art can produce them, will relieve and glorify those feelings in the act of expressing them. Catharsis is nothing more.

These stimulants to defecation are like the applications of the whip to the jaded horse-they excite the system to make a supreme effort in the required direction, but the reaction is disastrous in the extreme. With the repeated demands upon the delicate nervous system incidental to constant catharsis is it any wonder that we are so constantly confronted with cases of nervous collapse?
