The margin, while in parts irregular, is typically represented by a well-defined "rolled" border which consists of the peripheral portion of the cancer that has not broken down. The central ulcer may temporarily heal. There is itching but little pain, and the condition progresses extremely slowly; rodent cancers which have existed for many years are frequently met with.
In one of her letters she describes what was the daily scene: "Four at my feet listening; five boys outside getting a reading-lesson from Janie; a man lying on the ground who has run away from his master and is taking refuge until I get him forgiven; an old chief with a girl who has a bad ulcer; a woman begging for my intervention with her husband; a nice girl with heavy leglets from her knee to the ankles, with pieces of cloth wrapped round to prevent the skin being cut, whom I am teaching; and three for vaccination."
"Several years prior to his death the Dauphin, the father of Louis XVI., had confluent smallpox, which endangered his life; and after his convalescence he was long troubled with a malignant ulcer under the nose.
If the pain-lines about the mouth are of recent formation, and have not graved themselves into the furrows of the forehead above and between the eyebrows; if the color, instead of ashy, be clear and red, we throw out cancer and think of colic, ulcer, hyperacidity, or some milder form of alimentary disease.
In exceptional cases, blisters form over the seat of the effusion, or the skin may even slough, and the clinical features may therefore come to simulate closely those of an acute suppurative condition. When the skin sloughs, an ulcer is formed with altered blood-clot in its floor like that seen in scurvy, and there is a remarkable absence of any attempt at healing.
The areola was large and well marked, and 1/4 inch from its outer edge, immediately under the nipple, there was an ulcer with slightly elevated edges measuring about 1 1/4 inches across the base, and having an opening in its center 1/4 inch in diameter, covered with a thin scab.
She wished not to believe it, but her disordered corsage, her dishevelled hair upon her bare shoulders, her crumpled dressing-gown, and more than all that, the violent leaping of her heart, told her that she was not dreaming. He was gone, the priest; he had fled away into the night, happy and light of heart, leaving her alone with her shame, and the ulcer of remorse in her soul.
Many parts of his hands on the inside were chapped, and on the middle joint of the thumb of the right hand there was a small phagedenic ulcer, about the size of a large pea, discharging an ichorous fluid. On the middle finger of the same hand there was another ulcer of a similar kind.
Only in one form of surface-cancer, the so-called flat-celled or rodent ulcer, which has little or no tendency to form spore-cells and attack the deeper organs, are they effective.
C. Cocking, aged 17, has an ulcer of the size of half-a-crown on the inner part of the knee, occasioned by an accident. He had been a month under surgical care in the country when he applied to me, but the ulcer continued without disposition to heal, and fungous; it had apparently been treated by a solution of sulphate of copper.
