Needless to say, absolute rest is imperative. When all evidence of acute inflammation has subsided vesication is indicated. At this stage walking exercise is beneficial and the subject may be allowed the freedom of a paddock.
The application of the lunar caustic in recent burns or scalds, has always appeared to me to increase the inflammation and vesication, even inducing blisters where there were none before.
The older writers report some remarkable instances: Haller mentions a case lasting twenty-two weeks; Domonceau, six months; and Marcellus Donatus, six months. Whitelaw describes a boy of eight who, after an attack of scarlet fever, did not pass a single drop of urine from December 7th to December 20th when two ounces issued, after vesication over the kidneys.
Though one may strictly maintain that from our own experience we have a knowledge of formication, rapid and slow beating of the heart, even though we may not be able ordinarily to produce them at will, this position is absolutely untenable when we consider cases of vesication, stigmata, and other alleged miraculous phenomena: these are without precedent in the life of the individual.
#Clinical Features Local Phenomena.# The most generally accepted classification of burns is that of Dupuytren, which is based upon the depth of the lesion. Six degrees are thus, recognised: hyperæmia or erythema; vesication; partial destruction of the true skin; total destruction of the true skin; charring of muscles; charring of bones.
In this manner we frequently succeed in forming an adherent eschar, and all inflammation subsides. Any slight vesication which may be raised around punctured wounds is not of the same consequence as when an adherent eschar is wished to be formed over a sore or ulcer; one or more small punctures may be made to evacuate the fluid and the part may be allowed to dry.
7th: The inflammation began to advance. 8th: A vesication, perceptible on the edges, forming, as in the inoculated smallpox, an appearance not unlike a grain of wheat, with the cleft, or indentation in the centre. 9th: Pain in the axilla. 10th: A little headache; pulse, 110; tongue not discoloured; countenance in health. 11th, 12th: No perceptible illness; pulse about 100.
Vesication of the parts is in order, and this may be repeated in the course of two weeks, if it is deemed necessary. Penetrative wounds resulting in open joint are not treated with success as a rule, and because of the handicap under which veterinarians labor, methods of handling such cases, where large, important articulations are affected, are not being rapidly improved.
