The treatment consists in tiding the patient over the acute stage of his illness, until the fat is eliminated from the blood vessels. TRAUMATIC ASPHYXIA OR TRAUMATIC CYANOSIS. This term has been applied to a condition which results when the thorax is so forcibly compressed that respiration is mechanically arrested for several minutes.
In man it produces abdominal pain with diarrhoea and vomiting; dilated pupils, slow pulse, and cyanosis; delirium, insensibility, and convulsions. The post-mortem appearances are not characteristic, but the stomach and intestines should be examined for portions of the plant.
The gangrenous process spreads with appalling rapidity, the limb becoming enormously swollen, painful, and crepitant or even tympanitic. Patches of coppery or purple colour appear on the skin, and bullæ containing blood-stained serum form on the surface. The toxæmia is profound, and the face and lips assume a characteristic cyanosis. The condition is attended with a high mortality.
The Ephemerides cites an instance of triple heart, and Johnston has seen a triple heart in a goose. The phenomenon of "blue-disease," or congenital cyanosis, is due to the patency of the foremen ovale, which, instead of closing at birth, persists sometimes to adult life.
A distinctive murmur of this insufficiency would be diastolic and accentuated in the second intercostal space on the left of the sternum. It should be remembered that aortic murmurs are often more plainly heard at the left of the sternum. Sooner or later, if this lesion is actually present, the right ventricle dilates and cyanosis and dyspnea occur. Digitalis would therefore be indicated.
One of the latter cases was a sailor of sixty-three who suffered from general nigrities, and the other was in a woman of thirty, appearing after weaning and amenorrhea. Mitchell Bruce has described an anomalous discoloration of the skin and mucous membranes resembling that produced by silver or cyanosis.
The dyspnea is sometimes very great, and cyanosis may be present, especially during paroxysms of coughing. This lesion of the heart is always benefited by digitalis, but the continuance of the improvement and its amount depend, of course, on the cause of the dilatation of the ventricle. Strychnin is often of advantage.
There seemed to be no clue to the mysterious death of Dwight, not even a hint as to the cause. The most interesting point is the condition of the urine.
The affected area is sharply defined, and on close inspection the appearance is found to be due to the presence of countless minute reddish-blue or black spots, with small areas or streaks of normal skin between them. Pressure over the skin does not cause the colour to disappear as in ordinary cyanosis.
