There are cases on record where rapidly growing sarcomatous and angiomatous tumours, aneurysms, and the bruises that occur in hæmophylics, have been mistaken for acute abscesses and incised, with disastrous results. #Treatment of Acute Abscesses.# The dictum of John Bell, "Where there is pus, let it out," summarises the treatment of abscess.
In children who run about barefooted in towns, tubercle may be inoculated into wounds in the sole or about the toes, and although the local appearances may not be characteristic, the nature of the infection is revealed by its tendency to spread up the limb along the lymph vessels, giving rise to abscesses and fungating ulcers in relation to the femoral glands.
The swelling and inflammation of the throat, and the heavy, pus-like secretions that accumulate in the nasal cavities, cause difficult respirations. After a few days the abscesses usually break, and the symptoms are less severe. If the abscesses break on the inside of the throat, the discharge from the nostrils is increased.
Sometimes the nasal mucous membrane becomes affected, and produces a discharge at first watery, but later sanious and purulent. Necrosis of the bones of the nose may take place, in which case the discharge becomes peculiarly offensive. In nearly every case metastatic abscesses form in different parts of the body, such as the lungs, joints, or muscles.
Scrofula, which is another disease closely allied to consumption, is hereditary also; and hip disease, disease of the spine, abscesses, and enlarged glands in any members of a family, point to risks for the offspring which should not be forgotten, how much soever mental endowments, personal beauty, or the charms of disposition may be considered, and sometimes reasonably enough, to outweigh them.
It is certainly worthy of note that the uterus, peritoneum and intestines showed nothing special, but the liver was full of metastatic abscesses. At the exit of the hepatic vein from the liver there was pus, and its walls were ulcerated at this place. The pus from the liver abscesses was filled with the pyogenic vibrio.
A mixture of one part of mustard to two of linseed meal is, however, often of much use in the chest affections of children. Bread poultices are less generally useful than those of linseed meal. They do not retain the heat nearly so well as those of linseed meal, and are chiefly used in cuts, wounds, or small abscesses; and also because they are so easily made.
Ingleby gives another series of seven eases which occurred to a practitioner in 1836, the first of which was also attributed to his having opened several erysipelatous abscesses a short time previously.
