This is a hard, knotty condition of the udder, which sometimes follows calving, in consequence of the sudden distention of the bag with milk; and the inflammation which supervenes causes a congealed or coagulated condition of the milk to take place, of which, if neglected, suppuration and abscesses are the result. In more severe infections the exudate consists of pus mixed with fibrin, and, it may be, red blood corpuscles purulent or suppurative synovitis; the synovial membrane and the ligaments are softened, and the surface of the membrane presents granulations resembling those on an ulcer; foci of suppuration may develop in the peri-articular cellular tissue and result in abscesses.
Let the calf suck the dam as speedily as possible, and, if the hardness is not then removed, foment the udder with warm water; after which, wipe it dry, and apply to the entire surface melted lard as hot as the animal will bear. This is, generally, all that is required, the most obstinate cases yielding to it. If abscesses form, they should be lanced.
But I feel sure that, if I had resorted to ordinary dressing when the pus first appeared, the progress of the case would have been exceedingly different. The next class of cases to which I have applied the antiseptic treatment is that of abscesses. Here also the results have been extremely satisfactory, and in beautiful harmony with the pathological principles indicated above.
The bacteria which produce the various wound infections and abscesses, etc., appear to vary under normal conditions from a type capable of producing violent and fatal blood poisoning to a type producing only a simple abscess, or even to a type that is entirely innocuous.
Secondary abscesses in the lymph glands, subcutaneous tissue, or joints are often so insidious and painless in their development that they are only discovered accidentally. When the abscess is evacuated, healing often takes place with remarkable rapidity, and with little impairment of function. The general symptoms may be simulated by an attack of malaria. Prognosis.
Furthermore, it is often a terminal condition, and in such instances the results of treatment are of necessity nil. The most frequent terminal cause is nephritis; other terminal causes are pulmonary tuberculosis, adjacent abscesses, cancer or other growth.
While the breast is full and hard all over, not much apprehension need be felt. It is when lumps appear that the physician should be notified, that he may, if possible, prevent the formation of abscesses. While a woman is nursing she should eat plenty of nourishing food milk, oatmeal, cracked wheat, and good juicy, fresh meat, boiled, roasted, or broiled, but not fried.
In brief, it appears certain that every furuncle contains an aerobic microscopic parasite, to which is due the local inflammation and the pus formation that follows. Culture fluids containing the minute organism inoculated under the skin of rabbits and guinea-pigs produce abscesses generally small in size and that promptly heal.
