General dropsy, elephantiasis, lipomata, myxedema, and various other affections in which there is a hypertrophic change of the connective tissues may be mistaken for general obesity; on the other hand, a fatty, pendulous abdomen may simulate the appearances of pregnancy or even of ovarian cyst.
The most frequent forms are due to uterine congestion; to mechanical causes, as a narrowing of the cervical canal, particularly at its internal opening, or to a constriction caused by the bending over of the uterus at the junction of the body and the neck; or to ovarian irritation.
Milner Fothergill, in his book on Indigestion, vividly describes the appearance of the eyes sometimes seen in ovarian disorder: "The glittering flash which glances out from some female irides is the external indication of ovarian irritation, and 'the ovarian gleam' has features quite its own.
This fact would appear to be quite inconsistent with the theory above proposed, but it must be remembered that the ovum of Monotremes is known to remain for a short period in the oviduct, or in other words to pass through it very slowly, and to absorb fluid from its walls, as shown by the considerable increase in size which the ovarian ovum undergoes before it is laid.
Therefore, in each ovarian tube, in each glove-finger, the emergence of the eggs occurs according to the order governing their arrangement in the common sheath; and any other sequence is absolutely impossible. Moreover, at the nesting period, the six ovarian sheaths, one by one and each in its turn, have at their base an egg which in a very short time swells enormously.
The most unimportant of all organs would be the sexual organs, and the most essential peculiarity would consist in colour, which is to be referred back to the ovarian egg. "The embryos, or young states of different animals, resemble each other the more, the younger they are," or, as Johannes Muller expresses it, "they approach the more closely to the common type."
X-ray treatment and ovarian transplantation are the methods employed. As to human experiments, there is a colleague of Steinach's named Lichtenstern, who has operated on numerous men and women with apparent success. There has not been time yet to measure how long their new lease of life is to be; but they have regained the joy of life they had lost strength and powers of work.
The apex of the heart was pushed to the 3d interspace and the umbilicus had disappeared. There were 65 quarts of a thick, brown fluid in the tumor. The patient recovered in twenty-five days. Cullingworth of St. Thomas Hospital, London, successfully removed from a girl of sixteen an ovarian cyst weighing over 80 pounds. The patient was admitted to the hospital April 30, 1895.
An ovarian hematoma the size of a large orange was removed from the right side. We washed the intestines quite as one would wash linen, since some of the contents of the cyst had escaped into the abdominal cavity. The abdomen was closed without drainage, and the patient placed in bed without experiencing the least shock. Her recovery was rapid and uneventful.
To suppose that a secretion could be so complicated in its action as to determine each one of a thousand different items of structure, colour and behaviour would be preposterous. We may say that it cannot possibly be that the ovarian or testicular secretion is composed of enough different chemical substances to produce each different effect.
