The pyogenic infections and the exanthemata constitute the great majority of infections and are the basis of the discussion which follows.
#Secondary Hæmorrhage.# The term secondary hæmorrhage refers to bleeding that is delayed in its onset and is due to pyogenic infection of the tissues around an artery. The septic process causes softening and erosion of the wall of the artery so that it gives way under the pressure of the contained blood.
The axillary glands receive lymph from the arm, mamma, and side of the chest, and pass it on into the lowest cervical glands and the main lymph trunk. They are frequently the seat of pyogenic, tuberculous, and cancerous infection, and their complete removal is an essential part of the operation for cancer of the breast.
In the pyogenic infections, in order to protect the remainder of the body, which, of course, enjoys no immunity, every possible barrier against the spread of the infection is thrown about the local point of infection. How are these barriers formed?
Treatment. This is carried out on the same lines as in other pyogenic infections. In the earliest stages of the disease, the induction of hyperæmia is indicated, and should be employed until the diagnosis is definitely established, and in the meantime preparations for operation should be made.
A number of cases have been recorded in which arthritis deformans has followed upon antecedent disease of the joint, such as pyogenic or gonorrhœal synovitis, upon repeated hæmorrhages into the knee-joint in bleeders, and in unreduced dislocations in which a new joint has been established.
The liver is by far the most common site of hydatid cysts in the human subject. With regard to the further life-history of hydatids, the living elements of the cyst may die and degenerate, or the cyst may increase in size until it ruptures. As a result of pyogenic infection the cyst may be converted into an abscess.
Toxins are thus introduced into the blood, not only at the primary seat of infection, but also from each of these metastatic collections. Like septicæmia, this condition is due to pyogenic bacteria, the streptococcus pyogenes being the commonest organism found.
The pyogenic vibrio, found in the uterus, or which was perhaps already in the body of the mother, since she suffered from chills before confinement, produced metastatic abscesses in the liver and, carried to the blood of the child, there induced one of the forms of infection called purulent, which caused its death.
