If the rubber pessary has been properly fitted, and it is not porous, the protection should be complete; but if, by any accident, spermatazoa should get beyond the rubber pessary, they will be destroyed and tangled in the melted suppository provided, of course, that a suitable suppository has been used. It is all a question of getting the right articles to begin with and using them intelligently.

A doctor's help is needed only when first selecting the right size of pessary. The pessaries are made in ten different sizes, each size being numbered, and the right size can always be obtained on order.

When it is put up, let her lie with her legs stretched, and one upon the other, for eight or ten days, and make a pessary in the form of a pear, with cork or sponge, and put it into the womb, dipped in sharp wine, or juice of acacia, with powder of sanguis, with galbanum and bdellium.

But there is this chance a bare chance of accidental impregnation, and we want to eliminate all chances, if possible. Assuming the rubber pessary fits properly, as it will if skilfully selected and applied in the first instance by a competent medical practitioner, then the seminal fluid must remain in the lower part of the vaginal passage.

Take three drachms each of roast figs, and bruised dog's mercury; three drachms each of turpentine and duck's grease, and two grains of opium; make a pessary with wax. The room must be kept cool, and all motions of the body, especially of the lower parts, must be prohibited. Wakefulness is to be recommended, for humours are carried inward by sleep, and thus inflammation is increased.

The use of the rubber pessary is also sometimes unsuccessful because it does not fit properly, or because it is porous, or because in removing it some of the seminal fluid from the under-surface may be accidentally spilt in the vagina, and in this way the spermatazoa may later find their way upwards to an ovum.

Foment the privy parts with a decoction of the roots and leaves of dane wort. Take a drachm each of gum galbanum and opopanax, half an ounce each of juice of dane wort and mucilage of fenugreek, an ounce of calve's marrow, and a sufficient quantity of wax, and make a pessary. Or make a pessary of lead only, dip it in the above mentioned things, and put it up.

If you have mislaid the rubber pessary, a small sponge, a piece of clean cotton-wool, or even a piece of soft tissue paper can be used. Native women in different countries use seaweed, moss, sponge, etc., and Japanese women use rice-paper. But these articles are not so clean or effective as the occlusive rubber pessary.

Suppressing inflammation of the genital organs by poisonous antiseptics, sprays, tampons or other local applications only tends to aggravate the chronic conditions. Holding up the womb by means of a pessary in order to strengthen its muscles and ligaments is about as reasonable and effective as to try to strengthen a weak arm by carrying it in a sling.
