When adhesions have formed between the iris and the structures in relation to it, the pupil dilates irregularly under atropin. Although complete recovery is to be expected under early and energetic treatment, if neglected, iritis may result in occlusion of the pupil and permanent impairment or loss of sight.
Strychnia, brucia, ignatia, calabarin, woovarin, atropin, digitalin, and many others, including all whose effect is most tremendous upon the human system, are in this group. Not without insight did the early discoverers call nitrogen azote, "the foe to life."
A treatment as successful as any in this heart weakness in delirium tremens is morphin sulphate, 1/2 grain, and atropin, 1/15 grain, given hypodermically, with the administration of digitalis hypodermically for its later action on the heart. If the heart is contracting very rapidly, an ice-bag over the precordia will often quiet it.
Such irregularity perhaps most frequently occurs with valvular disease, especially mitral stenosis and in the muscular degenerations of senility, as fibrosis. Atropin has been used to differentiate functional heart block from that produced by a lesion. Jour. Med. In the first the heart block is induced by digitalis. This was entirely removed by atropin.
In speaking of the tolerance of belladonna, in 1859 Fuller mentioned a child of fourteen who in eighteen days took 37 grains of atropin; a child of ten who took seven grains of extract of belladonna daily, or more than two ounces in twenty-six days; and a man who took 64 grains of the extract of belladonna daily, and from whose urine enough atropin was extracted to kill two white mice and to narcotize two others.
Atropin in a dose of 1/150 grain, and strychnin in a dose of 1/40 or 1/30 grain are valuable aids in stimulating the circulation under these conditions. The atropin should not be repeated. The strychnin may be repeated in three, four or five hours, depending on the size of the previous close.
In these vomiting and collapse cases the hypodermic administration of morphin and atropin will not only stop the vomiting, at least temporarily, but will also give necessary rest. The dose of morphin need not be large, and the atropin may prevent nausea from the drug.
In a patient in whom we are trying to cause nervous and muscular rest, strychnin is certainly contraindicated. On the other hand, if the heart is acting sluggishly, the peripheral circulation is imperfect, and atropin is not acting well, it is advisable to give strychnin in a dose not too large and not too frequently repeated.
In the treatment of iritis the eyes are shaded from the light and completely rested, and the pupil is well dilated by atropin to prevent adhesions. If there is much pain, a blister may be applied to the temple. The Relations of Syphilis to Marriage.
His mind was occupied by another, more human problem. The discovery that the atropin and not the pilocarpin had been destroyed agitated him profoundly; not, as might be believed, because it enabled him at a critical time to regain the use of his sight, but because it threw before him an insistent question. Did, or did not, Bibi-ya-chui know?
