By applying the ear to the chest wall and listening to the lung sounds, absence of respiratory murmurs and abnormal sounds may be distinguished, due to consolidation of the lung tissue, abscess cavities and pleural adhesions. In a well-advanced case the hair is rough, the skin becomes tight and the neck thin and lean. The animal may breathe through the mouth when it is exercised.

Sometimes the blending of the two diaphragmatic structures, and consequently the severance of the one pleural duct from the abdominal cavity, is not completed in man. The two cavities then remain in communication by an open pleural duct, and loops of the intestine may penetrate by this "rupture opening" into the chest-cavity.

By his examinations he was able to prove that diseases of the chest, which had formerly been classed under the indefinite name "peripneumonia," might involve three different structures, the pleural sac covering the lungs, the lung itself, and the bronchial tubes, the diseases affecting these organs being known respectively as pleuritis, pneumonia, and bronchitis, each one differing from the others as to prognosis and treatment.

Krabbel mentions a patient who was run over by an empty coal car, and who died on the fifth day from suffocation due to an effusion into the right pleural cavity. On postmortem examination it was found that the effusion was chyle, the thoracic duct being torn just opposite the 9th dorsal vertebra, which had been transversely fractured.

Stelzner mentions a young student who attempted suicide by thrusting a darning-needle into his heart. He complained of pain and dyspnea; in twenty-four hours his symptoms increased to such an extent that operation was deemed advisable on account of collapse. The 5th rib was resected and the pleural cavity opened.

In case the pleural membrane is inflamed, the respiratory symptoms are more severe, and the hog shows evidence of pain when the walls of the chest are pressed on. The pericardium may be inflamed. In such cases the hog staggers and falls when forced to walk. The central nervous system may be involved by the inflammation.

One layer closely envelops the lung, at the apex of which it is reflected to the wall of the chest cavity of its own side, which it lines. In health the two pleural surfaces of the lungs are always in contact, and they secrete just enough serous fluid to allow the surfaces to glide smoothly upon each other.

An attempt to remove by the forceps failed, as the violent movements of the heart drew the needle back into the cavity. About this stage of the operation an unfortunate accident happened the iodoform tampon, which protected the exposed pleural cavity, was drawn into this cavity during a deep inspiration, and could not be found.

The pleural cavity subsequently filled with a dirty serum, but even this did not interfere with the healing of the wound and the restoration of the lung; the patient recovered without lateral curvature. Bartholf reports a case of rapid recovery after perforating wound of the lung.

It is to be inferred, as there were adhesions, that there had been pleurisy and deposit of lymph and serum, as in case 1, and that Nature had commenced the cure by absorbing the serum from the chest, and the lymph from the free pleural surface, and smoothed off every thing to a good working condition.
