"After the appearance of these symptoms, a more or less firmly adherent but limited perityphlitic abscess, and a less intense although well developed peritonitis in this region, were assumed; the latter, notwithstanding the painful meteorism, was not necessarily diffuse in the strict sense of the term; the omentum often protects the upper abdominal cavity from infection, as was proven in this case at the autopsy.
It is a common practice among many of the tribes to grease their weapons and implements with human fat, taken from the omentum, either of enemies who have been killed, or of relations who have died.
The stomach, small intestines, and part of the large omentum lay in the left pleural cavity; both the phrenic nerves were normal. Many similar cases of diaphragmatic hernia have been observed.
Mourray gives some notes on a case of crocodile-bite with removal of a large portion of omentum. Sircar speaks of recovery from a crocodile-bite. Dudgeon reports two cases of animal-bites, both fatal, one by a bear, and the other by a camel. There is mention of a compound dislocation of the wrist-joint from a horse-bite.
Walters saw the woman twenty-one hours afterward, and ligated and severed the protruding omentum. On the twenty-eighth day, after a marvelous recovery, she was able to drive to the Royal Berkshire Hospital, a distance of five miles. At the time of report, two years and six months after the mutilation, she was in perfect health.
The omentum was preternaturally yellow, and that part which covered the stomach was brownish. The external part of the stomach was extremely discoloured with livid spots; the internal part was extremely inflamed, and covered almost entirely with extravasated blood.
Fully one-third of the animals' weight comes under this head, that is, feet, tail, head, and tongue, lungs, liver, spleen, omentum, pancreas, and heart, together with the intestines.
The small intestines are suspended from the back wall of the cavity by a double fold of the peritoneum, called the mesentery. The bowels are also protected from external cold by several folds of this membrane loaded with fat. This is known as the great omentum.
In such cases the opening may be large enough to allow a great part of the visceral constituents to pass into the thorax, sometimes seriously interfering with respiration and circulation by the pressure which ensues. Alderson reports a fatal case of diaphragmatic hernia with symptoms of pneumothorax. The stomach, spleen, omentum, and transverse colon were found lying in the left pleura.
When this had been continued for some time, a portion of the omentum was cut off, wrapped in green leaves, and then put carefully away in a bag. The entrails were now replaced, a handful or two of green leaves thrust in above them, the cloths replaced, and the body again bound up ready for interment. Catlin, vol. i. p. 90.
