Beebe has shown that electric stimulation of the nerve supply of the thyroid diminishes the amount of iodin which it contains, and it is known that in the hyperactive thyroid in Graves' disease the iodin content is diminished.
An iodid circulating in the blood seems to aid absorption. It has long been believed that iodin in the blood tends to promote absorption of thickened, left-over material from exudates, and to prevent the formation of strong fibrous adhesions. Until our knowledge is more exact in this matter, it is advisable to use iodid as suggested. If the above-named dose is not tolerated, less should be given.
The margins of the wound were cleansed and the external wound enlarged to facilitate drainage. Tincture of iodin was injected; the wound was bandaged and dressed for a month in the manner heretofore described, when all discharge had stopped. The skin is cleansed and then painted with tincture of iodin. The sterile aspirating needle is pushed through the tissues and into the capsule with a sudden thrust.
It should not be used, except if specially indicated, in broken compensation or in other myocardial weakness. Iodids: These have no immediate action. The vasorelaxation that often occurs from iodid is quite likely due to the stimulation of the thyroid gland by the iodin, and the thyroid gland secretes a vasodilating substance.
The object sought by the introduction of iodin is not only for a local effect upon the synovial membranes in checking secretions, but the production of an active inflammation and great swelling, which will remain from four weeks to three months subsequent to the injection.
Whether this iodid or iodin acts per se, or acts by stimulating the thyroid gland to increased activity and therefore to more normal activity, so that it is the thyroid secretion which is of benefit, it is difficult to decide.
The object of such injections is to check synovial secretion, and this is sought by the local effect of iodin in contact with the secreting cells together with the reactionary swelling which occasions pressure. An increase in the local blood supply also follows. In all cases where it is possible to employ suitable bandages, this should be done.
