Infra-clavicular injuries, as already stated, are most often produced by a sub-coracoid dislocation of the humerus; the medial cord is that most frequently injured, and the muscles paralysed are those supplied by the ulnar nerve, with, in addition, those intrinsic muscles of the hand supplied by the median. Sensibility is affected over the medial surface of the forearm and ulnar area of the hand.
The lower head of the humerus remained somewhat enlarged; but the lameness became very slight, and in three weeks had nearly or quite disappeared. Nothing was done to the second fracture; in fact, nothing more than a slight, annular enlargement, surrounding the part, remained a proof of the renovating power of nature. II. He slowly improved with the exception of a pain in the left humerus, anteriorily, and in the upper part of the middle third. This became localized to a spot no larger than a twenty-five cent piece. At times the pain was intense and excruciating: and about a week from admission this spot seemed quite tender to the touch.
At its outer angle it has a shallow cup known as the glenoid cavity. Into this socket fits the rounded head of the humerus. The baby, usually under six months, is noticed to be feverish and fretful and to cry when touched. The mother discovers that the pain is caused by moving a particular limb, usually the arm, as the humerus, radius, and ulna are the bones most commonly affected; the limb, moreover, hangs useless at the side as if paralysed, and the condition was formerly described as syphilitic pseudo-paralysis.
Richardson speaks of a woman falling down a few weeks before her delivery. Her pelvis was roomy and the birth was easy; but the infant was found to have extensive wounds on the back, reaching from the 3d dorsal vertebra across the scapula, along the back of the humerus, to within a short distance of the elbow.
It may also be injured in fractures of the lower end of the humerus, in fractures of both bones of the forearm, and as a result of pressure by splints.
In the #infra-clavicular injuries#, the lesion most often results from the pressure of the dislocated head of the humerus; occasionally from attempts made to reduce the dislocation by the heel-in-the-axilla method, or from fracture of the upper end of the humerus or of the neck of the scapula. The whole plexus may suffer, but more frequently the medial cord is alone implicated. Clinical Features.
The forearm contains two long bones, the ulna and the radius. The ulna, so called because it forms the elbow, is the longer and larger bone of the forearm, and is on the same side as the little finger. It is connected with the humerus by a hinge joint at the elbow.
Amputation of the hip with recovery is fast becoming an ordinary operation; at Westminster Hospital in London, there is preserved the right humerus and scapula, presenting an enormous bulk, which was removed by amputation at the shoulder-joint, for a large lymphosarcoma growing just above the clavicle. The patient was a man of twenty-two, and made a good recovery.
