When these and other devices well known to every nurse are not effective, catheterization becomes necessary. With the elaborate precautions taken to avoid infection of the bladder, catheterization is now performed with very slight risk. Constipation, for various reasons, becomes a regular feature of the lying-in period.
A regimen that is nutritive and at the same time laxative is essential and in some cases cathartics and enemata are necessary. Also, during the first few days, if there is retention of urine, catheterization is imperative. In a word, the handling of such cases consists largely in keeping the subject inactive, as comfortable as possible, and giving attention to suitable diet.
A remarkable procedure recently developed by gynecologists, particularly by Kelly of Baltimore, is catheterization and sounding of the ureters. In the partial stupors it is seen as active opposition and cantankerousness. In the more profound conditions it is represented by muscular resistiveness or rigidity, or refusal to swallow food when placed in the mouth. Quite frequently a refusal to urinate is met with so that catheterization is necessary, or a patient may never use the toilet when led to it, but will defecate or urinate so soon as he leaves it. These latter, like some other perversities, suggest reactions of a petulant, spoiled child.
